Canonicalize external garden URLs to /-/https://… (#179)

* Canonicalize external garden URLs to /-/https://…

Legacy /-/host/path and collapsed https:/ forms permanently redirect to
/-/https://host/path. Repair collapsed schemes in canonicalize_item, and
always use the canonical path for resolver/pin next redirects.
